The forms below are required yearly for each returning student.
Re-Registration Form for the 2020-2021 school year.
Please complete the form by July 6th.
- CIAC (Connecticut Interscholastic Athletic Conference) Student and Parent Concussion Information Consent Form
- CIAC Student & Parent – Sudden Cardiac Arrest Plan and Consent Form
- Medication Policy, Agreement and Refill Medication Form – (for any student who will be taking Prescription and/or Over-the-Counter Medication while at school) – (Optional/If Applicable)
- Over-the-Counter Authorization Form
- Consent Form For Immunizations and/or School Physicals (For International Students)
- Athletic Permission Form
Please complete the following forms after July 15th and send to Health Office. These forms should be taken with the student when she goes to have her School Physical Examination.
